Harvi ECMO

Harvi-ECMO is an online training platform that includes two textbooks (Harvi-Student and Physiology of ECMO), an interactive simulation with advanced features related O2 and CO2 metabolism and guided tutorials related to extracorporeal membrane oxygenation (ECMO) and cardiopulmonary bypass (CPB).

The textbook also covers gas exchange (CO2 and O2) in the lungs, Oxygen Delivery, Fick Cardiac output, Control of Ventilation, the 5 causes of hypoxemia (Hypoxic gas mixture, hypoventilation, V/Q mismatch, Shunt, Diffusion abnormality) and basic interpretation of the arterial blood gas measurements. The text comes with eight try-it- now screens, each allowing the user to interact with important concepts and equations. An audio-annotated guided tutorial with self-assessment questions is also included.

Simulation Parameters

Harvi-ECMO simulation gives access to parameter sets to

  • ECMO and CPB device settings such as flows, gas sweep, inflow and outflow cannulae
  • Select from among 9 different cannulation configurations of ECMO
  • Patient characteristics such as alveolar minute ventilation, DLCO, hemoglobin, total body oxygen consumption and respiratory quotient
  • Initialize arterial blood gases

Simulation Outputs

Graphical output options include ability to display

  • Pressure-volume loops of 1, 2 or 4 chambers simultaneously
  • A wide range of continuous signal tracings over time (e.g., blood pressure, pulmonary pressures, blood flow profiles across any cardiac valve, LV and RV pressures and volumes, blood flow profile through shunts)
  • 1, 2 or 3 time plots simultaneously
  • Blood gases in each of 9 different compartments of the cardiovascular simulation (including arterial and venous blood gases)
  • Volume of blood reservoir during CPB along with inflow and outflow cannula blood flows
  • Ability to view the anatomy of each ECMO configuration

Numerical output options include ability to display

  • A wide ranges of standard cardiovascular parameters (e.g., blood pressure, pulmonary pressures, cardiac output, LV and RV ejection fractions, volumes and pressures)
  • Arterial and venous blood gases
  • Blood flow selected mechanical support devices
